The Tarlov Cyst Society has identified a spine surgeon in Indonesia who is actively treating patients with symptomatic Tarlov Cysts, marking a potential expansion of access to care in a region where options have historically been difficult to find. This development signals that Tarlov Cyst care is now available in Indonesia for patients seeking informed treatment options.
Dr. Yunus Kuntawi Aji, based at Mayapada Hospital Kuningan, shared details of the surgical approach he uses when treating selected patients.
According to Dr. Aji, the procedure involves a cyst wrapping technique. The surgery begins with a laminectomy to expose the cyst, followed by careful circumferential dissection of the cyst capsule. The cyst is then wrapped using artificial dura mater, punctured, and aspirated to remove its contents. The artificial dura is subsequently sutured to form a reinforced outer layer.
Dr. Aji said he considers surgery in patients who experience persistent radicular pain that does not respond to conservative treatment, as well as in cases involving neurological deficits, particularly those affecting autonomic function.
He reported that many patients experience symptom improvement following surgery, including relief from pain within days, improved quality of life, and the ability to return to daily activities. He also noted low recurrence rates at one- and two-year follow-up.
Dr. Aji indicated that he trained in this surgical technique under Prof. Yukoh Ohara at Juntendo University Hospital in Tokyo, Japan.
Tarlov Cysts, also known as perineural cysts, are fluid-filled sacs that form along nerve roots, most commonly in the sacral region of the spine. While often described and dismissed as incidental findings, many patients experience symptoms that can severely impact their daily lives.
